Wraparound Services in Santa Clara County
Post-Adoption and Wraparound Program Kinship Center offers intensive “Wraparound” services to children and youth at risk of placement in a high-level group home, and to their families. To be eligible for this program, the adopted child must be either 1) at risk of placement in a high-level group home or institution, or 2) presently placed in a group home or institution and is ready to transition home. The program is the result of a contract with Santa Clara Department of Social Services and is funded by Adoption Assistance Program (AAP) funds from Santa Clara County that otherwise would have paid for residential placement for the child. Each child and family is eligible to participate in the program for 18 months. The Kinship Center staff who are a part of this program have all received specific adoption related training and are highly qualified to work with the special needs of adoptive children. Wraparound Services in Montery County

To be eligible for this program, the adopted child must be either 1) at risk of placement in a high-level group home or institution, or 2) presently placed in a group home or institution and is ready to transition home. The program is the result of a contract with Monterey County Department of Social and Employment Services and is funded by Adoption Assistance Program (AAP) funds from Monterey County that otherwise would have paid for residential placement for the child, along with Mental Health Medi-Cal funding (EPSDT). Each child and family is eligible to participate in the program for 18 months. The Kinship Center staff who are a part of this program have all received specific adoption related training and are highly qualified to work with the special needs of adoptive children. Wraparound Services in Orange County
General Wraparound Program Kinship Center offers intensive “Wraparound” services to children and youth at risk of placement in a high-level group home, and to their families. To be eligible for this program, a youth 1) must have been adjudicated as either a dependent or ward of the Court and must be at-risk for placement in a high level group home licensed by the California Department of Social Services (CDSS), or 2) be on formal probation and referred by WRIT, or 3) be designated as a youth receiving AB3632 Special Education Services and be considered Emotionally Disturbed and at-risk for placement, or 4) be receiving Adoption Assistance Program funding (AAP) and be at risk because of qualifying behaviors of being placed outside the home. All referrals must be made by the WRIT Team. This program is a collaboration between Kinship Center and The Orange County Social Services Agency, Orange County Health Care Agency and Orange County Probation Department. Through this collaboration, a highly individualized approach is developed for each youth and his/her family. The goal is to maximize the capacity of a family to meet the youth’s needs, and to prevent or reduce residential placement, through the development of expanded family-based interventions. Adoption Wraparound Program

To be eligible for this program, the adopted child must be either 1) at risk of placement in a high-level group home or institution, or 2) presently placed in a group home or institution and is ready to transition home. The program requires an individual contract for each child between Kinship Center and the County from which the child was adopted. It is funded by Adoption Assistance Program (AAP) funds from this County that otherwise would have paid for residential placement for the child, and may also include Mental Health Medi-Cal funding (EPSDT). Each child and family is eligible to participate in the program for 18 months. The Kinship Center staff who are a part of this program have all received specific adoption related training and are highly qualified to work with the special needs of adoptive children.
